David Oliver: Base care on need, not age.

چکیده
In 2010 the NHS chief, David Nicholson, proposed a revolution in hospital inpatient care for older (not younger) people, akin to the closure of long stay psychiatric hospitals. But hospital admissions of older people continue to rise, and delays in discharge have worsened alongside fewer beds. One recurring policy idea that defies evidence and experience is that most “high need, high cost” older people can be kept away from hospital through lower tech, lower cost community interventions. 5 But, in describing inappropriate occupancy of acute beds by these pesky old folk who could hypothetically be cared for elsewhere, 7 soundbites are not evidence based solutions. Systematic reviews of evidence for reductions in admissions, bed occupancy, and cost, or for better outcomes resulting from care closer to home, have been far from conclusive. 9 The most recent promulgator of the idea was the surgeon and former health minister Ara Darzi, who wrote, “We need to keep high cost patients out of hospital.” His logic? A small percentage of patients account for a high percentage of hospital occupancy and spending. There’s no doubting that. Darzi described these patients as “typically older, with multiple chronic conditions” and “costing 400%more than the average.” How dare they? Darzi failed to mention that many also have acute illness or injury and rapid decompensation that complicate their frailty, as well as multiple long term conditions requiring an acute response. He didn’t propose keeping younger people away from hospitals in their hour of need.

David Oliver: The non-story of "NHS Airbnb".
As the NHS approaches its busiest time of year, politicians and the media are ramping up their focus on very real winter pressures. Overcrowded emergency departments, ambulances stacked outside, and patients on trolleys in corridors, are sure to make headlines. And don’t forget a pandemic scare: this year it’s Australian flu. No laughing matter, but hardly the only significant challenge to the ...

It’s a year since England’s health secretary, Jeremy Hunt, gave a speech linking his government’s promise of “truly seven day” NHS services with changes to contracts for the junior doctors and consultants supposed to enable this. He has suggested that a need for more “senior decision makers” (consultants) at evenings and weekends justified changes to trainees’ contracts. Meanwhile, however, the...

Study of the prevalence of incidental hamate hook fractures, nonunions, and other abnormalities on computed tomography (CT) ordered for another reason could document a subset of undiagnosed and untreated hamate hook fractures which might change our under...

In January the General Medical Council published its updated guidance on confidentiality and handling patient information. It’s a comprehensive manual highlighting doctors’ duties, backed by ethical frameworks, relevant law, good practice, and regulatory standards for most scenarios. 3 For jobbing clinicians, the most relevant section is “Disclosure to support the care of an individual patient”...
متن کاملDavid Oliver: Towards a GP consensus on the future of UK general practice.
As a hospital doctor, I rarely write about general practice unless it’s to defend, celebrate, or support it. Primary care is the engine room of the NHS and underpins its equity, efficiency, and access. But it’s underfunded, understaffed, and overstretched by rising demand. I’m breaking my “no primary care columns” rule and asking where GPs themselves think that UK general practice should go nex...
